Below are the digital downloads that go with module 6. This is to fill in your own birth plan. On the left is the fill in the blank birth plan for you to draft. That's what we discussed in the video above. On the right is the birth plan summary you can use as a template to give your OB or midwife and L&D staff.

This fill-in birth plan is for you to DRAFT your thoughts on the different options you'll have. This one is to help you make decisions by providing space on each topic. This way you won't miss anything.
For what you'll give your OB or midwife and L&D nurse, there is a separate 2-page template where you can summarize what you draft here.
Both templates are explained in my online class for birth plans.
